    I’ve been taking 1mg oral dutasteride aswell as tret+minoxidil and nizoral shampoo for 7 years and still losing hair. Should I increase to 2.5mg oral dutasteride daily as well as use topical finasteride?

    How is this possible? You’re removing hairs from the donor area, but the patient ultimately gains hairs in that donor area? Are new follicles being created in that area?

      We are regenerating significant portion of the donor area that is extracted. The hair that was not there before, but is apparent now is a result of transition telogen to anagen. A donor area can have 30% of the hair in the telogen phase which is dormant. If you were to turn all that hair back onto anagen, there would be a significant increase in density.

    How many millimeter punches do you use when removing hair grafts from the hairline?

      There isn't a one size fits all punch for hairline correction. If you have larger grafts placed along the front, you need a bigger punch and vice versa. The worst thing you can do is transect a graft you are trying to remove along the frontal hairline as it could end up buried and grow out worse than it started. Any physician that claims to only use one specific size punch is a moron and has not business performing hair restoration surgery. Our punch sizes range from .6mm-1.1mm. Look at coleinstruments.com Also note that our punches are true to size. Every other company that manufactures punches may claim to offer a .8mm. However, when we do a tricometer test on these companies punches, they are .2mm larger than what they advertise them being.

    Is minoxidil and finasteride meant to be applied for the rest of your life or can you get to a point where you can stop taking because your hair is where it needs to be and can stay that way without using treatment?

      if you stop them your DHT levels return to their normal state after 7 days and you will be back to your usual rate of hair loss before you started the medication, meaning you will probably lose the hair density you gained and might lose more existing hairs

    Would you be able to present that at the ISHRS or WFI? This is something that all surgeons should see and perhaps you can sell your secrets to other surgeons around the world.

      It is a good idea if the physicians would listen. Unfortunately, the equipment used to make this possible is expensive. Presenting this information would only open up an avenue for physician's to exploit patients and offer an inferior product. This would dilute patient confidence and only serve to hurt the consumer. Take PRP as an example, just about every physician offers PRP, but they purchase the cheapest devices and kits to process the blood. They often have no idea of the cell concentration they are injecting and usually do not produce a result. Today many patients that undergo PRP are skeptical because they hear all the bad reviews and lack of results from the industry. Though we have put on regenerative conferences in the past, few physicians wish to be apart of it though, it is the future of medicine in general. Take FUE as an example. This technique became available to physicians in 2004. It was hard pressed as the future of hair restoration surgery. It took over a decade for FUE to become popular and realized as the gold standard for hair restoration surgery today.

      @@forhair Yeah, that makes sense, but I think if you are truly able to get at least 30% regeneration and demonstrate it with adequate evidence, physicians would be willing to invest in it. The donor zone is otherwise a finite resource and people spend tens if not 100k+ on a transplant.if they're an advanced NW, so there clearly is demand. With even 30% regeneration, that basically means any patient without something like DUPA is cured.

      @@jeremyliak3480 We have been using a protocol since 2008 for donor regeneration that was enabling 30-60% follicle regeneration. It is not just the stem cells that is enabling regeneration but the procedure as a whole. In order to achieve this, exstractions must be minimal in circumference. They also need to be shallower. This will leave behind more tissue to enable regeneration. To do this you need tools designed for hair restoration. Unfortuntely many colleagues in the industry prefer to use adaptations of dental tools to perform the procedure. The devices are able to get grafts out, but they prevent any possibility of follicle regeneration. The less invasive the procedure, the greater the success rate is in terms of regeneration. Our new protocol now uses a mulitiude of growth factors. While I cannot speak on percentages at this time without more data, the results that have trickled in are better than what they were before.

      Hair will begin to grow at three months. You then have to wait for the hair to elongate. The transition is gradual and normal not to see noticeable regrowth until the 6 month mark.

      No, we are not able to achieve 100% regeneration. Our previous protocol took years to gather enough data to confirm 30-60% regeneration. Our new protocol is generating more regrowth, but we can't come out with a new bold numbers until the pool is expanded. The are evident extraction sites in the video where new follicles are not growing in. Another important factor to consider is much of the hair in the donor area wasn't visible prior to the procedure because they had cycled out and a new hair needed to populate. Our protocol stimulated those hairs into anagen. Hair that remains in the anagen phase will not shed. While it does require continued maintenance to prevent hair from cycling, continued treatments will prevent the need for a future hair transplant.

      Images were provided. There is a tattoo in the donor area to map the same location everytime. There are images before the procedure, immediately post op, and after regrowth. Even if you didn't believe the tattoo was in the same location. Compare the scars in the donor region to the result photos. There is clear pigmentation difference in some of the extraction sites. Though I understand a concern where you can find all the exstraction sites as the protocol does reduce scarring. The only way to improve on these images is to create a google earth scalp imaging layout to allow you to walk around the entire scalp.

He’s actually one of the best in the industry and has proven himself with not only the art of natural looking hair lines with minimal hair extraction being the best, but also tools.       The channel does not have a huge following. These are not claims. Pause the images. You can see the exact location of the tattoo. You can match the scars with the extraction sites, and you will see more hair in the donor area than was there prior to the procedure. This is factual evidence. You are one of the lucky few to see this result as information regarding cell based therapy is suppressed.

    When removing transplanted hair from the hairline, should I use FUE to remove it? Laser hair removal? Electrolysis? Which method leaves the least amount of scars?

      Attempting to laser the hair off usually does not work. It will also make the removal process more difficult. You are better off removing with FUE than attempting the laser.

      @@user-ib1ue2yk1j This is an excellent question. My first question to anyone looking to remove grafts is to understand their long-term goals. Do they want to remove everything and be naturally bald, do they want to have more hair and appear natural, or do they just want to remove the most apparent grafts? Removal of grafts is tricky and similar to changing money in a foreign country. When you give them $1, you do not receive $1 dollar's worth of foreign currency. You will receive less. You always lose something when you relocate grafts, as well. For this reason, I would like to know the goals. Most want to look natural and have more hair. In this instance, I remove only what I need to create a natural hairline. Because grafts from most clinics are not appropriately placed, the angle of orientation in the skin is guesswork. This means that some grafts will be destroyed upon removal. The goal is always to minimize the destruction of the hair in a graft, but we will eliminate some of the grafts. When the grafts are taken out intact, the survival rate is outstanding. When the graft is destroyed, the yield will be zero. So, we would like to remove only what needs to be removed and leave behind anything we do not need to remove. However, I can generally remove 90 to 99% of the grafts intact. One of the things I find from most doctors is that they do not get the grafts out intact, and they don't see and dig out the grafts they bisect. Repairing these repair cases is even more complex because they changed the exit angle and left scars to contend with.

      The removal process is not scarless, but the scarring is minimal and often require magnification to see. Another concern is pitting as a result from the prior bad hair transplant. Removal may not resolve the pitting issue. Though the reason this area heals so well is there is more blood flow to the front of the scalp than the donor area which reduces scarring.

      There are a few prinicples at play here. First, the size of the wound is smaller than 1.5 mm so the scarring will be minimal. The second is that the hairline region heals better than the back of someone's head so scarring on the hairline and the beard area are minimal. This principle is also why hairline advancement procedures heal well, but it's not a good idea in people (males in particular) with hairloss. There will be a scar in the males so stick to FUEing the grafts out when possible. Another reason is the punches I make for myself make smaller wounds and follow langer's lines.

Anecdotally, have you noticed better results with topical Fin vs Dut? Lastly, would you recommend that a patient apply the topical Fin or Dut at same time as topical Min?

      It would be easier to apply both treatments at the same time. The end result won't vary if you apply one first followed by another later.

      yes. you should first check if there are other reasons for your diffused thinning (ie: thyroid, hormones etc,,,)

    Me personally, I'm a 45 year old man using estradiol/estriol cream (a cent size daily), 5 mg oral finasteride, 5 mg of oral minoxidil, monthly kenalog shots and my hair is regrowing. The temples are filling in and vertex is looking full. I'm going to start using oral spironolactone soon and this I believe will give me the full hairline restoration. Hair cloning is not necessary if the follicular ostium are all present, it's just a matter of using the right therapies. I don't care frankly if I transition into a woman like body, as long as the hair is thick and can grow long is all that matters for me personally.

      You are not the first person I've seen so focused on their hair. Be careful with the kenalog because it can shut down your adrenal glands. Monitor your cortisol levels. The estrogen can effectively chemically castrate you and protect your hair. You might also look at bicalutamide. Could you keep me posted on your progress? You are correct that as long as you have active hair follicles, you have a shot at re-growing them.

      5mg of fin/min ? that's' a bit overkill right those are the prescribed doses for other medical issues

      @@itschasezilla ​ @chasejoseph Actually, you're right about that. However people my age, 40+, need a higher dose of finasteride, 2.5 to 5 mg to be effective, based on my reading. I'm doing 150 mg a day of oral spironolactone and seem to be fine. It's helping me be more regular too so that's a plus as I'm trying to lose a few pounds.

      The protocl we have has improved. This protocol looks in the Acell preventing it from seeping out which will improve the result. Also, are able to minimize scarring with our Zero T punch and should have a video coming out on the wound size. Lastly, our incisions are at a shallower depth which mitigates scarring. This combination of factors is a less invasive procedure, but anytime the body is penetrated to create a wound a scar is left behind.

      you commented on a different video in which you applied "regenera" , it is a different product, that we don't use when we say "regenerative medicine" it means a variety of products like CRP, exosomes, stem cell therapy etc...

    From 2013-2020 I maintained hair well with generic oral finasteride and topical rogaine. After covid I stopped maintaining. I stayed on same protocol for 2 years still losing ground daily. In 2023 I switched to oral generic dutasteride and oral minoxidil (5mg)/day bit after a year, I'm still noticeably thinner and continue to lose ground. Since this protocol isn't working, I'm either going to switch to brand name oral Finasteride (brand Dut too expensive) or topical versions of both. Thoughts? Any input is appreciated.

      There are many medications out there to combat hair. You may want to consider a genetic test to see what you respond best to. Name brand is a better product than generic. We do tend to see better results from topical treatments, but not everyone has the receptors to dissolve certain medications. All these treatments are designed to slow down hair loss, but they will not stop it. You could also try mesotherapy in conjunction with take home medical therapy to enhance the results.

    I have been using XYON 2.5% topical finasteride with 3% minoxidil gel daily for the past 1.5 months. After the first 2 weeks I noticed some sides such as loss of sex interest and more importantly loss of morning erection (which I used to have daily before starting the topical gel). To exclude the psychological effect, I kept using the gel daily hoping things would change if it was only in my head. Unfortunately, It’s still the same sides. My concern is that this 2.5 % is super high dose (=25 mg finasteride on scalp). So even if very little absorbed, say 5% of the 25 mg, it means 1.25 mg goes systemic which is like an oral dose). Any insight or feedback ?! On a side note, not sure if it’s of any relevance, two weeks after starting the gel, I experienced problems in the two arms joints (shoulder and elbows) little pain and stiffness especially at night. This never happened to me before so I wondered if it’s related to the finasteride or it’s just a coincidence!!

      Try going as low as .5 % finasteride with 3% minoxidil. There is still a risk of systemic side effects. Other than the loss of morning wood, are you still able to get it up if you try? Regarding joint pain, the is the first reported case I have come across. Have you changed anything else in your daily routine?

      @@forhair thanks for your kind reply. Actually loss of morning wood has been a major concern for me. However, I’m not 100 sure if it’s nocebo effect, especially with my bad experience with oral propecia years back. I can still get erection from time to time but libido is low, again I’m not sure if it’s only in my mind. That’s why I keep using it hoping that my mind clears up. Yes, I do not believe that propecia causes this joint pain. However, I was concerned coz this pain (which I never had) started less than two weeks after starting the topical formula. Before starting the topical one 1.5 month ago I was using nutrafol supplement for 1 year (it has multivitamins and collagen). I stopped it just right before I started the topical finasteride, not sure if this caused a drop in collagen, vit D, etc that affected my joints, or something else….

      @@user-pz8vo7wh6l I don't react well with Minoxidil and I use the 2.5% Xyon. Try applying it every 3 days to give your body a chance to clear some this will still allow your hair to be saved and not overloading your body. This is especially important while your body adapts.
